What is jerk four vector? and what is its definition or a form? as far as I know it is a rate of change of 4-acceleration.

I have never seen a jerk four-vector, but if you took the four-acceleration and differentiated wrt proper time then you would have something that is manifestly a four-vector. I would probably call that the four-jerk.
