For example, in the US, medical schools are expensive, so people studying to become medical doctors usually have to fund it by taking out large amounts of loans. They then spend the first part of their working career paying off $100,000 to $200,000 (or even more) debt. I suspect this is not true in some other countries.
On the other hand, again in the US, physics PhD students normally receive enough financial support from their universities to cover tuition and basic living expenses. They usually do have to pay for the bachelor's degree which comes before this, but so do the medical students.
